Calling someone by a title can be seen as respectful or distant, depending on how someone feels about it. Using him as an example again, Dimitri'd rather not be called by title.
I'm not sure who might have been in charge of everyone as a whole. The colonel was in charge of the arbiters, the military had many people with different titles, Madam Lu I think was in charge of the Garden of Eden...Mister Polly was in charge of the research institute...is a prince similar to that?
A prince is the son of a king, and a king's in charge of all the other guys in charge. Top of the food chain, you could say. Princes become kings, though the circumstances around that may vary. Dimitri will one day be a king.
The thing about being at the top though is getting to be treated like anybody else becomes a luxury. At least, I'd say that's how Dimitri feels about it.
He notices aroma and texture more than most, so I expect foods that provide neither just don't do it for him.
